Synopsis

Two years after the events of the original game, Sergeant James Heller, returning from a tour in Iraq, discovers that his wife and daughter have been declared dead, leading him to join the military in NYZ's fight against the Blacklight Virus. Heller learns of previous outbreaks in Idaho in the 1960s and in New York during the events of the first game. He later finds Mercer and begins chasing him.

Pursuing Mercer for revenge, Heller is infected by Mercer with a strain that grants him similar superhuman abilities. After losing consciousness, Heller wakes up in a Yellow Zone laboratory where Gentek scientist Dr. Anton Koenig and Blackwatch Colonel Douglas Rooks are experimenting on him. Heller escapes from Gentek's base, when Mercer confronts him and claims that Gentek and Blackwatch are responsible for recreating and cultivating the Blacklight Virus, making them responsible for the second infection and the death of Heller's family. Revealing his plans to eliminate Gentek and Blackwatch, Mercer offers Heller a truce. Unsure of Mercer's intentions, Heller turns to his local pastor, Father Luis Guerra, for advice and help.

Using information provided by Guerra, Heller hacks Blacknet terminals to discover and sabotage a number of their operations. Gradually making his way through Blackwatch, Heller eventually finds and confronts Koenig, who claims to be on his side and reveals Blackwatch's super-soldier program, codenamed "Orion". Heller prevents the progress of Project: Orion by killing a super-soldier who was injected with his DNA. After devouring one of the leading scientists, Heller discovers that Koenig has been observing him in order to discover his weaknesses. Enraged by Koenig's betrayal, Heller tracks him down to discover that he has powers similar to those of him and Mercer, and that he is one of several "evolved" agents implanted in Gentek and Blackwatch by Mercer. After defeating and devouring the doctor, Mercer reveals to Heller that he intends to recruit him to try to control NYZ. With his doubts growing, Guerra then shows Heller a videotape of Mercer releasing the virus for the second time at Penn Station, the same location where Mercer initially released and contracted the virus.

Enraged by Mercer's deception, Heller travels to the Green Zone to track down Mercer's henchmen, including an evolved Gentek agent named Sabrina Galloway; However, after confronting Galloway, Heller reluctantly teams up with her when she reveals that she can help him defeat Mercer. With Galloway's help, Heller soon discovers that Mercer plans to infect the entire world with "Whitelight", a contaminated vaccine released by Gentek which actually serves to accelerate the infection and its progression rather than prevent it.

Frustrated that Heller is sabotaging his plans, Mercer kills Father Guerra; After finding his corpse, however, a grieving Heller uses Guerra's phone to reach Athena, Guerra's contact who turns out to be Dana Mercer. Dana explains that Heller's daughter Maya is still alive, prompting Heller to head to the Red Zone to take out Mercer and save Maya. After preventing Blackwatch's second attempt to raze the island of Manhattan, Rooks helps Heller reach his daughter, only for Galloway to betray him and take Maya to Mercer. Upon confronting Mercer, he reveals that he plans to solve international conflicts and global problems by infecting the entire human race, creating a super-organism with Maya at the center. After Mercer absorbs Galloway and the remaining Evolved, the two engage in a bloody duel, only for James to emerge victorious and kill and consume Mercer.

Subsequently, Heller eliminates the infected in New York as well as most of the Blacklight viruses; then using Mercer's absorbed memories, Heller locates and frees Maya and Dana from a vault. The story then ends with the three overlooking New York before Dana wonders what to do next.
